Volunteer Forms

Thank you for your generosity in applying to partner with survivors of Hurricane Sandy in their recovery.

Prior to arriving at your assigned work site, there are several forms that we need you to complete (see chart below) and insurance you may need to secure (below the chart). All of the applicable forms are included on this page.

Please save each form pdf to your computer, complete all fields, print, sign and have notarized, where applicable. You can save the pdf and come back to complete it at a later time, if you wish.

NYAC Participant Liability Release Form

NYAC Team Leader Checklist — this document covers information about what you should expect when volunteering, a Team Leader Checklist and includes the items that you should bring/wear. Please pay close attention to this information.

NYAC Mission Covenant

NYAC Parental Consent Release/Medical Form (for youth) — after completing this form, please have the parent/guardian signature notarized. Most town/city halls and area banks have a notary on staff. A copy of this form should be kept by the team leader, as well.

Insurance

Non-Methodist teams will need to provide a $1M certificate of liability insurance in the name of the church/facility where they are staying.  Send certificate to Gina Grubbs at the address above at least two weeks in advance of scheduled trip.
